Purpose

This form gathers qualitative and quantitative feedback about safety, clarity, inclusion, and transformation.

It fulfills three roles:

  1. Ethical: gives members voice and agency.
  2. Operational: informs event improvements.
  3. Legal: documents that Tribal Loves actively monitors cultural health (fulfilling PMB Duty of Care).

All responses are confidential and stored in the Culture Pulse Database (Notion) under role-based access.
